Your responses allow us to review your information and prepare recommendations before determining the appropriate next steps.

If appropriate, you'll receive a separate medical intake directly from our licensed medical provider. Any prescription therapies, including peptides or GLP-1 medications, require an independent medical evaluation and approval by the provider.
